What to Bring to Your First Appointment

To ensure a fast and efficient registration process, please provide our administrative staff with the following:

  • Completed new patient form
  • Photo identification (driver's license, state ID, or passport)
  • Updated personal information
  • Updated insurance card
  • Current medication list with dosages and frequency details
  • List of primary care doctors, cardiologists, referring doctors, chiropractors, and physical therapists, including addresses and phone numbers

If you're coming for a consultation related to a work injury, please bring the following:

  • Primary Treating Physician (PTP) details
  • Information about your workers' compensation carrier, nurse case managers, and/or adjusters

Medical Records & Imaging for Your First Appointment

To ensure Dr. Hieu Ball has an accurate and complete picture of your condition, please bring the following:

  • Information about your referring care-providers, including telephone, fax, and address so that all copies of our visits can be sent to all of your care-providers to keep them informed about your care.
  • Copies of all pertinent medical records from prior physicians, chiropractors, physical therapists, etc. 
  • Copies of imaging studies, including MRI Scans, CT Scans, and X-rays, should be done within a year of your appointment. They should be brought in on a CD or actual sheets of film. Also, please provide a copy of the official report from the radiologist or doctor who performed the study or test.
  • Copy of your Operative Report if you have had previous spine surgery.

Bringing these documents and records to your appointment will help us provide you with the best possible care and ensure an efficient consultation. Having all of the records readily available will help the efficiency of our clinical consultations and prevent delays and respect your time, as well as that of other patients.

What to Expect During Your Appointment

New Patient Visits

For a new patient, the first visit will involve a comprehensive evaluation. Dr. Ball will start by reviewing your medical history and discussing any current symptoms, lifestyle factors, and concerns you may have. A complete physical exam will be conducted to assess your overall health and spinal function.

In some cases, if you haven't had recent imaging studies (such as X-rays, MRIs, or CT scans), Dr. Ball may order digital imaging during your appointment to help with the diagnosis and treatment planning.

Follow-Up Visits

During follow-up visits, Dr. Ball will continue to work closely with you to track your progress. He will review any tests or imaging results from prior visits and discuss the findings with you in detail.

Whether it's evaluating the effectiveness of your current treatment plan or making adjustments based on your symptoms, Dr. Ball will take the time to explain his recommended course of action.

To ensure an efficient and stress-free visit, we recommend you keep the following in mind before your appointment:

  • Arrive 15 minutes early to complete any necessary paperwork.
  • Wear comfortable clothing, especially if you’ll be undergoing a physical exam.
  • Bring a trusted friend or family member for support, if needed.
  • Allow approximately 60–90 minutes for your visit, as Dr. Ball spends quality time with each patient.

While we strive to maintain a manageable patient schedule that respects your valuable time during our clinical consultation days, there are times we might run behind. Occasionally, some patient encounters require unanticipated additional time to review an MRI properly or thoroughly answer questions leading up to a surgical procedure.
